I have been doing intermittent fasting for a number of years, but recently started increasing my fasting times. Due to health issues, I tend to be nutrient deficient. When I do 20/4 fasting, that four hour window is not enough time for me to eat enough to get the nutrition I need (I don’t have much of an appetite after one meal), but I feel really good fasting for 20+ hours at a time. Will fasting still be effective if I eat for a longer period, but still fast for 20+ hours in between?

You could do an alternate day fasting style. Essentially start fasting say sunday night, fast through monday and then eat all day tuesday. You can also do what you are thinking with longer eating periods but that will get in the way of sleep at some point. You can also just mix things up and do omad some days while a more traditional 16/8 on other days.
One thing to also do is look at liquid based nutrition. Like break a fast with a bone broth. Then use something like a smoothie to get some protein and nutrients. Those are liquid and should pass through the stomach quicker. Just experiment and find little things that work for you.
I can not eat enough veggies and other fiber on omad. So switched backed to alternate day eating or a 24 hour fast where I break it right before bedtime so I’m not hungry at night.
My body feels best eating light throughout the day versus one meal then occasionally doing a 36 hour fast.
Omad makes me feel like I’m bingeing and I eat more crap foods. But it totally works for others. Just find out what’s best for you.
