Intense nausea and hunger pangs in the morning while fasting?

I’ve been fasting for a little over a year. I typically do 16:8 but if I’m feeling okay I will go over and occasionally do 20 hour fasts but that’s usually as long as I go. I’m obese and not sure if I’m pre-diabetic or anything. I need to go get that checked.

But in the mornings (and sometimes randomly at other times of the day, but almost always in the morning at least) I will get these moments of hunger pangs followed by intense nausea. It usually lasts for a few minutes. It’s pretty uncomfortable to be really, really hungry but also feel like if you even think about food you might throw up. It’s just this wave of discomfort and I’m not sure if it’s normal or how to avoid this. Could it be low blood sugar? My mom claims to be hypoglycemic so it’s possible I could be as well, though I don’t know how you work around that with IF. Curious if anyone has any ideas or experiences with this. Or perhaps I should just schedule a doctor appointment.

Checking with a doctor is a good plan. If you believe this is related to your blood glucose levels get a monitor and check throughout your day for a week to see what ranges you are in. This could be a number of issues. Perhaps electrolyte and water balance. It could also be digestive peristalsis and liver enzymes and bile production. Doc could get you set up with correct course for correcting the issue though.
