28 days is a very long time, and you need to be very careful while breaking the fast, especially the first week.
Thia is just a layman summary but essentially when you are fasting, your cells get depleted but your blood tries to preserve an acceptable nutrient balance. Then as you start eating again, the cells sense food coming in and start drawing it from thr blood stream. I ghink phosphorous was one especially important part that needs to stay stable in the blood, but if there isn’t enough entering your blood stream while the cells are absorbing it from the blood, it may run low in the blood which can be quite dangerous, so you need to make sure you are getting enough phosphorous into your blood along with electrolytes.

Considering your condition, I would seek out medical treatment again, and make sure they run blood tests to measure the blood nutrients, especially electrolytes and phosphorous, and preferrably I suppose take a phosphorous supplement.

Whatever it may ve, it seems like a medical emergency to me.
