Ok, given your history here are some thoughts:

  1. Do you have enough spare weight for a 40 day fast? You’re talking about a 20+ lb weightloss – not counting water weight and loss of gut bacteria/enzymes.
  2. I get rather cold about a week into a fast. Be ready with sweaters, heavy socks, and adjusting the thermostat. I got chilly even when I was obese.
  3. Good electrolyte supplementation will make the entire process much easier. The FAQ gives some solid guidance, but felt like a bit too much for my taste. In any event, you’re beyond the “add a pinch of table salt to your water” range of fasting.
  4. At times during the fast I felt rather bad. Be willing to give up and stop. (If you’re wondering why I didn’t, there were serious health issues I had to make progress on in a restricted time window. I’m also married to an MD and had frequent bloodwork). Depending upon how much of a worrier you are, a weekly CMP is not expensive and checks a bunch of things.
  5. Your feelings and weight loss will not be consistent. I had a period of a week where I felt very bleah, but then felt tremendous on days 29, 30, and 31. (31 days is my longest; I can’t speak beyond that)
  6. Getting some sun exposure really helped. Days when I could sit on the deck in my underwear were usually good days. Normally I dislike the sun as I burn easily, so it those results were surprising. Not sure if vitamin D or something else.

Also, take seriously the comments from other folks: try a shorter fast first – perhaps 14 to 21 days. You’ll experience many of the above issues, but won’t be digging as deeply into your body’s reserves.
